Risk of falling objects due to pieces being ejected from GT exhaust stacks?

• Have any users experienced objects or material being ejected from a turbine stack, such as internal liner plate, retention washers, battens etc?
• If so, which turbine model, and what action was taken?
• Has any member retrofitted mesh at the stack exit or similar provision to address the risk of falling objects?
